1. Here's a list of clickable links to go to the client page - I've not been very imaginative with the numbering ;-) Opening the page will increment a counter associated with the CPT.  This is for your testing purposes only - I didn't bother with setting up your form for this demo.

2. Go to the Show Results page from the menu to see the output from the shortcodes to display results on front end.  They output tables, and I have embedded plenty of classes so you shouldn't have any trouble styling.
